You will only receive a confirmation if we can accommodate your request to play.

If you already registered, the team leader should have received a confirmation of registration. Then entire team then would have each received an email confirmation of the processing of the registration.

KEY CONSIDERATIONS

• Formal invitations will be sent for the exact date/time to physicians at their office address on file. Please be sure the Edward Medical Staff Office has your current office address.

• RSVP’s are REQUIRED to attend this event.

• Space is limited. Physicians must reserve either a foursome, twosome, or a single spot by providing exact RSVP information. This includes all team member names and contact information.

• Reservations are limited to EDWARD Hospital Physician Medical Staff Members. Guests are not included in this event due to space limitations. Ancillary staff, non-physician staff members, and other Edward healthcare members are not included due to space limitations.

• Invitations are non-transferable.

• Food will be served outside. Cocktail reception/awards is held at the Woodside Pavilion, an open-air covered area.

• Physician team leaders MUST fill out their team registrations and make sure team members are aware of the team submission to prevent duplicate registrations.

• Verbal/Voice Mail RSVPs are not be accepted. All RSVPs must be in writing and returned via written RSVP or online registration.

• For the quickest response to any questions, please email EDW-docservices@outlook.com. We cannot take phone calls regarding events. There is insufficient staff to handle the volume of calls we receive.

This is a physician-only event. If you are not an MD, DO, DDS, DMD, DPM, or PsyD, please do not attend this event. Physicians, do not bring office or clinical staff to this event. This applies to all physician events. Space at these events is limited and is reserved for physician members of the Edward Medical Staff.
