2nd Annual Kasai Fire & Fairways Charity Golf Tournament: Scottsdale Silverado Golf Club | Monday June 05, 2023
*it is your responsibility to understand these rules set forth by the insurance company. These are insurance industry standards for the majority of golf contests. Contests similar to those offered at Fire and Fairways Charity Golf Tournament events.
This agreement obligates Golf Planner Pro LLC doing business as Perfect Golf Event (PGE) to indemnify the Client, for whose exclusive benefit this agreement is made, up to the specified maximum Prize Value if any hole in one occurs on the Date(s), Hole(s) and Tournament as specified in accordance with the USGA Rules of Golf. PGE’s payment obligation is conditioned upon full compliance with the following terms and conditions.
BEFORE THE TOURNAMENT:
DURING AND AFTER THE TOURNAMENT:
• _For prize values up to $9,000 and Bonus Hole in One Prizes, playing partners will be acceptable as witnesses.
• _Greater than $9,000 and up to $50,000, one (1) third party witness must be stationed at the green or tee box of the Designated Hole to witness all attempts
• _Greater than $50,000 up to $75,000, two (2) third party witnesses must be stationed at the green or tee box of the Designated Hole to witness all attempts
• _Greater than $75,000 up to $99,000, three (3) third party witnesses must be stationed at the green or tee box of the Designated Hole to witness all attempts.
• _Greater than $99,000 up to $1,000,000, three (3) third-party witnesses, one being a PGA Pro or Assistant Pro, must be stationed at the green or tee box of the Designated Hole to witness all attempts AND all attempts must be videotaped in full with such video providing a raw, unedited video record of each participants complete path of the ball. If a PGA Pro or Assistant Pro are not available, a General manager, superintendent, or Police Officer shall serve as the third third-party witness.
_An affidavit from the potential winning contestant attesting to his/her eligibility and compliance with the Official Rules;
_A copy of the potential winning contestant's drivers license;
_An affidavit from the designated witness(es) as well as each of the potential winning contestant’s playing partners attesting to the circumstances of the hole-in-one;
_An affidavit from a representative of the Client attesting to compliance with the terms and conditions of this Insurance;
_A copy of the Videotape of the insured attempt (for Hole-In-Ones with prize values greater than $99,000);
_A copy of the original scorecard reporting the Hole-In-One occurrence, signed by the winner;
_A list of the players who participated in the contest;
_A letter from the Golf Pro on letterhead stating the hole #, yardage that was played, the tournament name, date, and number of golfers.
This agreement obligates Golf Planner Pro LLC doing business as Perfect Golf Event (PGE) to indemnify the Client, for whose exclusive benefit this agreement is made, up to the specified maximum Prize Value if a made Insured Putt attempt occurs on the Date(s), Hole(s) and Tournament as specified in accordance with the USGA Rules of Golf. PGE’S payment obligation is conditioned upon full compliance with the following terms and conditions.
BEFORE THE TOURNAMENT:
1. In order to book coverage, PGE must receive full payment prior to the event date.
2. The Client must provide all needed information, such as the hole number (practice green is allowed) where the Insured Putt attempt will take place.
3. PGE will email a coverage confirmation based upon the information submitted by the Client. The coverage confirmation will supersede any prior agreements or representations, and contain the full agreement of all parties.
4. Any changes in the hole number, distance, number of attempts, prize value, etc., require notification to PGE two business days prior to the tournament date. Once the change has been accepted by PGE, the Client will receive a revised coverage confirmation.
DURING AND AFTER THE TOURNAMENT:
1. GOLF COURSE SPECIFICATIONS - The Insured Putt must take place on the green of a regulation hole at a regulation golf course (practice green is allowed). The Putting surface may not be specially prepared or altered from the condition which is usual for normal play on such hole nor shall the cup be positioned on the green outside of where it is normally positioned in order to facilitate a Putt. All equipment and the hole/cup must adhere to USGA or PGA specifications.
2. DISTANCE REQUIREMENTS - The minimum distance must be 50 feet, unless prior approval is given by PGE. The Client may choose a higher distance for the Insured Putt. Measurements must be made from the front edge of the cup to the Putt origination point.
3. REGISTERED PARTICIPANT AND NUMBER OF ATTEMPTS - This Insurance will not cover a contestant who is a current or former professional golfer or a current or former NCAA Intercollegiate Golfer that has played in a NCAA Intercollegiate Golf tournament within the past five (5) years, unless prior approved by Underwriters. The participant must be a registered participant in the insured tournament. No practice attempts or mulligans are allowed at any time at the Designated Hole on the day of the event. The number of total Insured Putt attempts cannot exceed the listed number on the coverage confirmation.
4. QUALIFICATION PROCESS - It is understood that a qualification process may take place in order to determine which registered participant will attempt the final Insured Putt. It is further understood that the qualification process may take place on the same hole used for the final Insured Putt. This does not constitute a practice shot as long as the final Insured Putt originates from a different location on the green. The Client cannot designate the Putt location until right before the final Insured Putt takes place.
5. WITNESS/VIDEOTAPE REQUIREMENTS - A third party witness must be at least 18 years of age and cannot be a participant in the insured tournament.
• _For all Putting Contests four (4) third party witnesses are required. Two (2) will confirm the measurement of the Putt length, and two (2) third party witnesses will be stationed to witness the attempted Putt.
• _The Insured Putt attempt must be videotaped in full with such video providing a raw, unedited video record of the participant’s complete path of the ball.
6. IN THE EVENT OF A SUCCESFUL PUTT - The Client must contact PGE within the next business day. The Client must provide Underwriters with the following information as proof of claim:
• _An affidavit from the potential winning contestant attesting to his/her eligibility and compliance with the Official Rules;
• _A copy of the potential winning contestant's drivers license;
• _An affidavit from the designated witnesses attesting to the circumstances of the Insured Putt;
• _An affidavit from a representative of the Assured attesting to compliance with the terms and conditions of this Insurance;
• _A copy of the Videotape of the insured attempt.
Those that purchase tickets (max one ticket / 1 attempt per player) for the putting contest will putt from 45' at the practice green as they round the course during game play (between holes: 18 & 1). The 3 closest to the pin during the tournament will have a putt-off at end of the tournament or 12:45pm at the practice green (have your cell phones on**). At the Putt-Off, you will attempt from 45' (one attempt - no practice shot). The person closest to or in the hole wins the chance for a single attempt from 50'. Sink it and $5,000 is yours!
*One Person Will Win The Chance To Putt From 50 Feet - 1 (One) Person Wins 1 (One) Attempt Only. MUST BE VIDEO TAPED BY NON PLAYING / PARTICIPATING PERSON.
**We will make three attempts to reach you on your phone. If we cannot contact you via your phone number, you will forfeit your Putt-Off attempt.
***Winner needs to be present to win. No substitute players. No practice shots.
****The Pin Remains In-Hole during all attempts including, putt-off, and chance for $5k.
KASAI'S FIRE AND FAIRWAYS TOURNAMENT HAS PURCHASED CONTEST ITEMS FROM: Golf Planner Pro LLC doing business as Perfect Golf Event (PGE). The rules above are directly from PGE. Rules and terms of service may change without notice per PGE.
