Fees & Terms

• Event registration fees:
  • $85.00 Golf and Luncheon. Fees include:
    • Vera Bradley Gift, Donation to Vera Bradley Foundation for Breast Cancer, greens fees, lunch & cart rental.  Valued at $155.00
       
  • $60.00 Luncheon Only.  Fees include:
    • Vera Bradley Gift, Donation to the Vera Bradley Foundation for Breast Cancer and lunch.  Valued at $105.00
       
• Payment options include check or credit card.  Please note: when paying on-line by credit card, a convenience fee of $2.50 will be charged for each registrant.  If paying by credit card, we encourage the use of Visa and Mastercard.
   
• All CCI members and their guest(s) registration fees will be charged to CCI account. Please select payment by "check" option at checkout.    You will NOT need to actually issue a check, registration fees will be added to your CCI account.
   
• Non CCI members, if paying by check, please print email registration confirmation and mail with check made payable to Golf Fore a Cure to:

• Payments and registrations must be received by June 10, 2016
   
• No refunds or rainchecks. In the event of severe weather, the golf scramble will be cancelled, however the luncheon will take place as scheduled. 
   
• Registrations are transferable.  Please contact Jayme Plude at CCI (317) 291-9770 ext. 201 with new attendee information.
