PAYMENTS and CANCELLATION POLICY - Execution of this sponsorship registration form signifies assumption of legal responsibility to pay for all sponsorship requirements as stipulated above. Payment must be made in US dollars and drawn from a US bank. For tax reporting purposes the Guide Right Foundation of St. Louis, Inc. is a tax-exempt 501(c)(3) organization. Tax ID Number: 15220826

If you are registered but can no longer play, full refunds will be granted, per golfer, if notice is given by June 21. After June 21, we will reserve $25 as a $100% tax deductible donation to the Guide Right Foundation of St. Louis, Inc. and refund the remainder upon written request.
