All contests winners must meet eligibility requirements.  Each contest must be managed according to the contest guidelines outline below and on the insurance contracts, which will be available for review at the tournament or by appointment at the offices of Secure One Capital.

Secure One Capital Corparation and Caterina's Club are not responsible for items lost or stolen while attending or participating in this event. Secure One Capital Corporation, Caterina's Club, tournament sponsors, nor their employees or volunteers are responsible for any loss due to injury, theft, or act of God. 

In the event of inclement weather, civil unrest, wild fire or other natural disaster, reprentatives of Secure One Capital, Caterina's Club and Strawberry Farms Golf Club will determine if the tournament should be rescheduled and will choose a sililar date in the future. Players unable to attend scheduled date may request a refund or transfer thier registration to another golfer.

CONTEST RULES

Million Dollar Hole-in-One Shootout

This is a shootout. Videotaping of shot(s) is required from a location adjacent to the target green and all contestants must sign the official sign-in form provided by AHNO. Two (2) witnesses are required, one positioned at the tee and the other at the target green. One (1) of the official witnesses must be either a PGA-certified professional or law enforcement official. If qualifying rounds are used to determine the finalist(s), total yardage must be a minimum of 15 yards different from the qualifying round(s) yardage and the pin placement may be required to be changed. No shots may be attempted from the yardage listed on the contract by an official contestant on the same day as the grand prize attempt. It is the responsibility of the client and/or sponsor to ensure that signage on display communicates to potential or actual finalist(s) that the prize is not available during any preliminary rounds or tournaments. AHNO may begin sign production prior to actual receipt of signed contract, so please review contract details to assure accuracy. Please review all terms & conditions and the responsibility affidavit with tournament director. Thank you for your business.

WARRANTIES AND CONDITIONS
1. SIGNED CONTRACT & PAYMENT - A signed contract (two pages) and payment by check , credit card, or wire transfer must be received by American Hole 'n One, Inc. (AHNO) prior to the day of the tournament. Coverage is not provided without payment and signed contract. For Rush Orders, AHNO will accept a Credit Card Hold (Visa, MasterCard, or American Express) as payment, along with a faxed, signed contract, until your check has been received.
2. CONFIRMATION OF TERMS - AHNO will Fax, or email written confirmation on all telephone communications prior to the tournament. Only a signed and completed AHNO application received prior to tournament date will supersede prior communications of terms and conditions. Once a contract is established, any changes to the contract must be provided by AHNO in writing.
3. CONTESTANT ELIGIBILITY - Prize coverage fee charges are based upon three categories of shots: those taken by amateurs; those taken by professionals (as defined by USGA guidelines); those taken by PGA, LPGA and foreign touring professionals. You must correctly state the number of shots to be taken in each category to have a valid contract. For example, for a tournament in which professionals are playing with amateurs and are not designated as ineligible to win hole-in-one prizes, the number of each type of contestant must be clearly stated on this contract. If a professional golfer is eligible to win any hole-inone prize(s), the individual must be counted as a participant in this contract. All players must be officially registered competitors in the specific event named on this contract.
4. FORMAT OF EVENT - The event covered on page 1 of this contract must be a golf shootout. A shootout is defined as an event with any number of shot(s) taken at a golf course, independent driving range, or other custom location outside of the normal round of play of a golf tournament. The format of the shootout, qualification of contestants, and custom event location (if applicable) is subject to approval by AHNO.
5. WITNESSES - All official witnesses must be non-participants, age 18 or older, and appointed by sponsor or responsible tournament official as follows: Prize limits to $99,999 require two witnesses, one of whom is positioned at the tee-off area and the other of whom is positioned adjacent to the hole, out of harm’s way. For prize limits of $100,000 or more, two witnesses are required, positioned in the same manner as described above, one of whom must be a PGAcertified professional or law enforcement official.
6. VIDEOTAPE - For shootouts, regardless of prize value, must be video recorded. In the event of a claim, the raw, unedited video recording of the winning shot must be submitted to AHNO. The recording must show the exact position of the golfer in relation to the tee, the actual swing of the golfer, the flight of the ball (as best as may be captured), the landing of the ball on the green, the ball dropping into the cup and remaining in the cup, and the ball being removed from the cup. The person recording the shot should be positioned behind the green, filming toward the tee area. Recordings that are not one continuous filming with edits may not be valid. It is the responsibility of the named client on page one to verify that all recording equipment is functional,
including adequate battery power.
7. YARDAGES - The target hole(s) will measure from the tee to the flagstick, no less than the specified yardage on this contract. Women may shoot from a maximum of 15 yards less than men, without affecting the contract rate; however, in no event may any contestant shoot at the main prize hole(s) from less than 135 yards.
8. NUMBER OF SHOTS - Each shot taken for the target hole must be accurately stated on the contract.
9. CHANGES TO THE NUMBER OF SHOTS - Changes to the number of shots, without regard to the reason or the number of shots to be taken, must be called in to AHNO prior to the shootout. You may communicate any changes in required coverage via phone at 800-822-2257 (outside of normal business hours you can reach our emergency 24/7 phone or leave a message in AHNO's Emergency Mail Box) or via fax at (770) 271-4007, or contact AHNO’s general
email inbox at info@ahno.com. It is imperative that adjustments to the number of shots are notified prior to the start of the tournament, as adjustments after the day of the tournament may not be awarded.
10. MULLIGANS & PRACTICE SHOTS - Absolutely no practice shots or mulligans are permitted on the target hole(s). Mulligan is defined as tournament or player providing or purchasing a do over of a shot. Mulligans may not be used to win prizes and are strictly prohibited.
11. CANCELLATIONS – If the tournament is cancelled for any reason, including weather, assuming no shots were taken on the target hole(s), a full refund less a $50 cancellation fee will be made, or this contract will be amended to a rescheduled rain date without additional charge. AHNO must be notified of any cancelled or rescheduled shootout prior to or within 24hrs from the date of the tournament.
12. TARGET HOLE – Only one predesignated hole may be used on the target hole green. Coverage does not apply unless prize is offered on the EXACT target hole as specified in this contract.
13. SHOOTOUT REQUIREMENTS – If you are hosting a shootout and the main prize hole is used for qualifying shots or tournament play on the same day as the shootout, the yardage for the shots must vary by a minimum of 15 yards from that used for the qualifying rounds or tournament play. All shooting activity toward other nearby greens, flags or cups must be suspended and all golf balls must be cleared from the green and the surrounding area prior to the commencement of the contest and to each subsequent golfer's shot.
14. PRIZE RESTORATION – Unless otherwise specified, AHNO will award the prize specified for every ace during a shootout. If an individual prize value exceeds $100,000.00 AHNO will have a (1) winner maximum for that listed prize.
15. PRIVACY – AHNO will not release any information regarding shootout to any other parties other than client listed on page one unless authorized by client. The names and likeness of Sponsor and winner may be used by Sponsor and/or AHNO for promotional purposes without further compensation to the Sponsor or winning contestant.
IN THE EVENT OF A HOLE-IN-ONE
16. CLAIM NOTIFICATION –Immediate telephone notice by sponsor to the claim department of AHNO must be reported no later than the first business day after shootout. Fax, mail or email AHNO the name, address, and telephone number of the winner, the official witnesses, the club pro, as well as the raw unedited videotape recording (if applicable).
17. DISPUTED CLAIMS – AHNO acts specifically in behalf of the sponsor named on this contract and specifically not as agent of any competitor in a tournament in which AHNO provides coverage. Any disputes are subject to the jurisdiction of the courts of Gwinnett County, Georgia with each party liable for their own attorney’s fees. Sponsor specifically waives the right to trial at any venue other than Gwinnett County, Georgia even if Sponsor’s state law can claim jurisdiction in the event of a dispute with AHNO. Don’t risk having a claim denied by neglecting to follow the simple terms and conditions of this
contract. *Be sure to have payment and signed contract received prior to the date of the event. (Check or credit card accepted). Sponsor must provide accurate yardage figures of your main hole-in-one prize(s) and an accurate count of amateur, professional and PGA/LPGA touring professional golfers.

at, the golf course named on page 1 of this contract.
5. WITNESSES - All official witnesses must be non-participants, age 18 or older, and appointed by sponsor or responsible tournament official as follows: Prize limits up to
$99,999 require two witnesses, one of whom is positioned at the tee-off area and the other of whom is positioned adjacent to the hole, out of harm’s way. For prize limits of
$100,000 or more, two witnesses are required, positioned in the same manner as described above, one of whom must be a PGA-certified professional or law enforcement
official.
6. VIDEOTAPE - For golf tournaments with prizes valued $100,000 or more, all shots must be videotape recorded. The videotape recording must show the exact position of the
golfer in relation to the tee, the actual swing of the golfer, the flight of the ball (as best as may be captured), the landing of the ball on the green, the ball dropping into the cup
and remaining in the cup, and the ball being removed from the cup. The person videotaping the shot should be positioned behind the green, filming toward the tee area.
Recordings that are not one continuous filming with edits may not be valid. It is the responsibility of the named client on page 1 to verify that all videotaping equipment is
functional, including adequate battery power.
7. YARDAGES - The main prize hole(s) will measure from the tee to the flagstick, no less than the specified yardage on this contract. Women may shoot from a maximum of 15
yards less than men, without effecting the contract rate; however, in no event may any contestant shoot at the main prize hole(s) from less than 135 yards. These distance
requirements are intended as requirements for main prize offerings for which AHNO is providing coverage.
8. BONUS PRIZES - AHNO provides bonus prizes to enhance the tournament in which coverage is provided. These prizes must be placed upon the exact holes as indicated in
this contract. No independent witnesses are required on these holes as AHNO will accept the word of two playing partners if an ace is made and a copy of the contestant’s
scorecard. Bonus prizes shall be defined as being additional prizes offered by AHNO as a consequence and benefit of sponsor purchasing a hole-in-one contract from AHNO
for their main (i.e. car, cash, boat, etc.) prize(s). Bonus prize holes may be played from normal men’s and women’s tees, however no bonus prize may be played from less
than 100 yards.
9. NUMBER OF SHOTS - For tournament play, each category of shots specified on this Contract permit a 5% variance, plus or minus (+ or -), without a change in the contract
fee. Shot variance greater than 5% must be reported to AHNO prior to the tournament. Coverage fee adjustment will be billed or refunded after the event. IMPORTANT: The
prize value will be prorated downward if a hole-in-one occurs and the number of shots has been understated by more than the allowed 5% variance. (Example: Number of
shots covered divided by number of shots taken times the prize value = Amount Paid.)
10. CHANGES TO THE NUMBER OF SHOTS - Changes to the number of shots, without regard to the reason or the number of shots to be taken, must be called in to AHNO prior
to the tournament. You may communicate any changes in required coverage via phone at 800-822-2257 (outside of normal business hours, call AHNO’s emergency phone or
leave a message in AHNO's Emergency Mail Box) or via fax at (770) 271-4007, or contact AHNO’s general email inbox at info@ahno.com. It is imperative that adjustments
to the number of shots are notified prior to the start of the tournament, as adjustments after the day of the tournament may not be awarded.
11. MULLIGANS & PRACTICE SHOTS - Absolutely no practice shots or mulligans are permitted on the main prize target hole(s). Mulligans may be sold on the bonus prize holes;
however, only the first shot taken by each contestant is eligible to win the hole-in-one prize offered. A golfer who is 'rounding out' a foursome by taking an extra shot for the
team is only eligible on his or her first shot. Only the authorized number of shots (plus or minus the 5% variance as indicated above for tournament play) are specifically
permitted during the stipulated round(s) of the named tournament on the date(s) listed in this contract. Mulligan is defined as tournament or player providing or purchasing a do
over of a shot. Mulligans may not be used to win prizes and are strictly prohibited.
12. CANCELLATIONS – If the tournament is cancelled for any reason, including weather, assuming no shots were taken on the target hole(s), a full refund less a $50 cancellation
fee will be made, or this contract will be amended to a rescheduled rain date without additional charge. AHNO must be notified of any other cancelled or rescheduled
tournaments prior to or within 24hrs from the date of the tournament.
13. TARGET HOLE – Only one predesignated hole may be used on the target hole green. Nine (9) hole courses must specify which hole(s) will be eligible during the official
covered round. Coverage does not apply unless prize is offered on the EXACT target hole as specified in this contract. Holes designated 1-9 would be considered front side
and holes 10-18 the back side of a nine hole course. Tournaments using more than two sets of nine-hole courses (i.e. 27-holes or 36-holes) must clearly indicate the name of
the specific course(s) to be played, the main prize and/or bonus prize hole numbers on those courses, and the number of shots to be taken on each of those holes.
14. PRIZE RESTORATION – Unless otherwise specified, AHNO will award a prize for every ace on a contest hole shown on page 1 during that specific tournament. If an
individual prize value is $100,000.00 or greater AHNO will have a (1) winner maximum for that listed prize.
15. PRIVACY – American Hole ‘n One will not release any information regarding the tournament to any other parties other than client listed on page 1 unless authorized by client.
The names and likeness of Sponsor and winner may be used by Sponsor and/or AHNO for promotional purposes without further compensation to the Sponsor or winning
contestant.
IN THE EVENT OF A HOLE-IN-ONE

CLAIM NOTIFICATION – MAIN PRIZE - Immediate telephone notice by sponsor to the claim department of AHNO must be reported no later than the first business day after
tournament. Fax, mail or email AHNO the name, address, and telephone number of the winner, the official witnesses, a playing partner, the club pro, as well as the raw
unedited videotape recording (if applicable). In addition to the aforementioned information, we will also require the original (or clear photocopy) of both sides of the winner’s
scorecard and the original tournament pairing sheet.
17. CLAIM NOTIFICATION – BONUS PRIZE – Immediate telephone notice by sponsor to the claim department of AHNO must be reported no later than the first business day
after tournament. Fax, mail or email to AHNO the name, address and telephone number of two playing partners. AHNO reserves the right to substitute bonus prizes of the
same type and of equal or greater value. (See definition).
18. DISPUTED CLAIMS – AHNO acts specifically in behalf of the sponsor named on this contract and specifically not as agent of any competitor in a tournament in which
AHNO provides coverage. Any disputes are subject to the jurisdiction of the courts of Gwinnett County, Georgia with each party liable for their own attorney’s fees. Sponsor
specifically waives the right to trial at any venue other than Gwinnett County, Georgia even if Sponsor’s state law can claim jurisdiction in the event of a dispute with AHNO.

Important execution requirements per the terms & conditions include, but are not limited to:
YARDAGE - Signage provided for your tournament by AHNO states the prize and displays a hole number and minimum yardage. The main prize hole(s)
must play from the minimum yardage listed on the contract and printed on the prize sign(s), regardless of tee box locations. IMPORTANT: It may be required
to move the tee boxes to the correct yardage
2. WOMEN GOLFERS - On main prize hole(s), women may play from a maximum of 15 yards closer than men to a minimum distance of 135 yards, regardless
of tee box locations. IMPORTANT: Women's tee boxes may be required to be moved to the correct distance prior to the start of the tournament. (On bonus
prize hole(s), women may play from the ladies tees.)
3. HOLE # - The signs provided by AHNO must be displayed on the correct hole number as stated on the contract. Tournament officials are responsible for
confirming the prizes are displayed on the correct hole numbers. There is no valid prize coverage for a main prize placed/displayed on an incorrect hole
number.
4. WITNESS REQUIREMENTS - Two non-playing witnesses (aged 18 or older) are required on each main prize hole. One witness must be positioned on the
tee and one on the green in a location that allows the witness to clearly see the rise and fall of the golf ball and the ball go into the hole. All eligible hole
in one shot attempts must be in the presence of the witnesses to be eligible for prize coverage. The tee witnesses must be able to verify that every golfer
shoots from the minimum distance required per the contract. (Bonus prize holes do not require witnesses.)
5. NO MULLIGANS - No mulligans or practice shots are allowed at any time on a main prize hole. Each golfer may have a maximum of one attempt
regardless of the outcome of the shot. A golfer who is 'rounding out' a foursome by taking an extra shot for the team is only eligible on his or her first shot.
(Mulligans may be taken on bonus prize holes, but prizes are only awarded for their first shot.)
6. AMATEUR GOLFERS ONLY - Hole in one prize coverage is valid for amateur golfers only (according to USGA guidelines), unless declared prior to
tournament and stated on contract issued by AHNO.
7. NUMBER OF GOLFERS - AHNO must be notified prior to the tournament if the number of golfers varies by more than 5% from the number of players listed
on the contract. (A maximum of 5% variance is permitted without contacting AHNO.)
8. TOURNAMENT PLAY ONLY - Prizes and signs are for tournament/event play only. Signs should be placed at the holes immediately prior to the event and
should be removed after the final player has played a par 3 hole. No claims will be considered for players who are not part of the contracted tournament or
event.
9. EVENT CANCELLATION - If the event is canceled or rescheduled, you must contact AHNO as soon as possible and no later than the first business day
following the originally scheduled event date.
10. PRIZE CLAIM FOR A HOLE IN ONE - If any eligible golfer successfully makes a hole in one on any prize hole, immediate telephone notification must be
made to the AHNO claims department (800-822-2257) no later than the first business day following the event. A signed affidavit and/or polygraph test by a
professional polygraphist may be required from witnesses and/or tournament administrators to verify the facts of this claim.

$25,000 Helicopter Ball Drop

The Helicopter Ball Drop is an exciting way for people all over the world to participate in this outstanding fund raising event for Caterina's Club.  Anyone may purchase a chance to win. You may purchase as many chances as you like. There is no limit on the number of chances to win.  Various pricing packages are available.

Each chance will be assigned a number. We will be using 1,000 balls, due to helicopter capacity and time restraints.  We will also be using color codes.  When we drop the numbered balls, we will also be dropping colored balls, with each color being assigned to 1,000 chances. (for example: yellow = 1 - 1,000; green = 1,001 - 2,000 and so on)  This is how it works. Let's discuss chance # 1237, for example.  Since we are using only 1,000 balls, if ball number 237 is in the bottom of the cup and the green ball/or bean bag is closest to the cup then the person who purchased chance # 1,237 would be declared the winner and win up to $2,500. (based on 33% of total sales, with max guaranteed prize being $2,500, there is no minimum guaranteed prize.).   Then, a representative of Secure One Capital and Caterina's Club will open a sealed envelop.  If the number in the sealed envelope is 1,237 then the person who has that number will win $25,000!!!

Contest managers/represntatives will not have access to the list of players or their numbers at the time of the event.  Once a winning number is determined, the contest managers will reach out to GolfBallDroppers.com to determine the person who matches the winning number.  

In the case that the ball at the bottom of the cup does not match a player, we will record all balls, which are in the hole/cup to determine a winner.  Represntatives of Caterina's Club and Secure One Capital will contact the winner on Monday, September 18, 2017. 

The entire Golf Ball Drop and ball identification will be recorded and may be broadcast live or recorded on KFI AM 640.

2. This contract is null and void without a signed copy of the contract and payment received at least 24 hours in advance of
contest event date.
3. Exhibit A, the official Promotion rules are a part of this Contract and shall remain in effect in their entirety during the term of
the Contract.
4. In the event that the actual conditions of the Promotion are materially different from those represented by the Sponsor/Client,
this Contract shall be null and void and any prize coverage fees forfeited unless such changes are made with express written
consent of AHNO.
5. American Hole ‘N One, Inc. is not a party to the Promotion or conduct of the Promotion and Sponsor/Client shall indemnify
AHNO from any claims made because of Sponsor/Client(s) misimplementation of the Promotion. This contract contains no
provisions for prize fulfillment which may arise from any causes other than for reimbursement of the specified prize value.
6. All copyright, trademark and other property rights currently owned by the Sponsor/Client or AHNO shall remain the property of
said owner with each party to this contract giving due respect and notice to such ownership, and each party to the contract
retaining all legal rights and enforcement powers inherent in the ownership of said intellectual property.
7. The aggregate liability of AHNO shall be limited to the sums payable under the terms contained herein.
8. SHOULD A DISPUTE ARISE BETWEEN SPONSOR/CLIENT AND AHNO AND/OR BETWEEN CONTESTANT(S) AND
AHNO, BOTH PARTIES SPECIFICALLY WAIVE THEIR RIGHTS TO ANY LEGAL JURISDICTION TO SETTLE SUCH
DISPUTE(S) IN ANY VENUE OTHER THAN GWINNETT COUNTY, GEORGIA. SPONSOR/CLIENT ACKNOWLEDGES
RESPONSIBILITY TO MAKE CONTESTANT(S) AWARE OF THIS REQUIREMENT.

American Hole ‘n One, Inc. (AHNO) agrees to reimburse sponsor for the value of scheduled prizes awarded in conjunction with sponsor's promotional contest. Participants shall be eligible to receive prize in the amount of $25,000.00 CASH in the event that the winning requirements outlined herein are met in explicit accordance with the manner prescribed by the official rules, terms and conditions that follow. 

This contract is void without a signed copy of the contract and payment received at least 24 hours in advance of the contest event date.
1. Contestants for this event will be those who purchase one of the 1,000 Golf Balls sold by the client. There must be a minimum of 1,000 numbered Golf Balls used for this event.
2. Each of the Golf Balls will be dropped from a helicopter onto a designated hole (i.e 18th green) on the golf course. The Golf Balls must be numbered from 001 to 1,000. In order for the $25,000.00 CASH prize to be awarded, the number printed on the Golf Ball which come to rest closest to the pin or inside the cup of the designated hole on the golf course (in specific accordance with all terms and conditions outlined herein) must match exactly with the (1) number (001-1,000) pre-selected by AHNO (see items 3a-c below) and contained in the sealed prize envelope.
3. Important: the following procedure will take place concerning the administration of this event:
a) AHNO will pre-select the (1) winning number between 001 and 1,000. The (1) winning number will be placed inside a sealed prize envelope and shipped to a representative of the client prior to the start of the event. This envelope must remain sealed at all times during the promotion. Once all Golf Balls have been dropped from the helicopter and have come to rest on the designated golf course, a client representative must call an AHNO representative to report the number which is printed on the Golf Ball which has come to rest closest to the pin or inside the cup of the designated hole on the golf course. This will be the only Golf Ball eligible for the grand prize, and no other Golf Ball(s) or numbers will be acknowledged for the purposes of this contract.
b) In the event that the number(s) on any of the Golf Ball(s) which come to rest closest to the pin or inside the cup of the designated hole on the golf course does not match the (1) number pre-selected by AHNO, the AHNO representative will direct the client representative to open the sealed prize envelope and reveal the (1) pre-selected number to the contestant(s).
C) In the event that the number(s) on any of the Golf Balls which come to rest closest to the pin or inside the cup of the designated hole on the golf course does match the (1) number pre-selected by AHNO, the AHNO representative will direct the client representative to return the unopened sealed prize envelope to AHNO. The envelope must be received by AHNO in its original sealed and pristine condition. If the sealed envelope has been opened or damaged in any way by any individual other than a representative of AHNO, there will be no opportunity for a claim for this event.
4. Important: All Golf Balls must be allowed to fall and roll freely and naturally within the boundaries and surrounding areas of the golf course once they are released from the helicopter. Nothing may be done to any of the Golf Balls (or the area within or surrounding the designated golf course) that would in any way effect the natural path of the dropped Golf Balls or enhance any individual's chance of winning the $25,000.00 CASH prize.
5. It is imperative that the sealed prize envelope containing the winning GOLF BALL number be kept in a secured location by the designated official until directions are given by an AHNO representative. If there is no winner in the event, an AHNO representative will inform the official holding the sealed prize envelope that the envelope may be opened for verification of the official winning number. In the event that the selected number matches the predetermined winning number on file at AHNO, the envelope must be returned to AHNO and must be received in its original sealed, unmolested condition. The envelope will be examined thoroughly by AHNO upon receipt. The envelope seal may not be broken in any manner, nor may there be any tears, creases, or other marks visible on the envelope. Any signs of tampering on the sealed prize envelope will result in an immediate claim denial. Nothing may be done in any way to enhance a contestant's chances of winning this contest.
6. Important: once the sealed prize envelope has been opened, irrespective of the reason, no claims shall be acknowledged for this event.
7. Important: it is the responsibility of the contest supervisor to ensure that the selected number(s) are reported correctly during the phone call (see item 3a-c), and that the selected number(s) are reported to an AHNO representative. Once it is determined that there is no winner in the contest, and the winning number is released, there will be no further opportunity for a claim in this event. American Hole ‘n One will not be responsible for a Golf Ball number that is not reported correctly to an AHNO representative and confirmed at the time described in items 3a-c above. If a winning Golf Ball for any reason is not reported prior to the release of the official winning number(s), the contest supervisor/client will assume all liability and American Hole ‘n One will not be responsible for paying any claims.

8. American Hole ‘n One is not responsible for any miscommunication between a contestant and the contest supervisor or between the contest supervisor and an American Hole ‘n One representative. Only the selected number(s) reported during the phone call to an AHNO representative (as described in item 3a-c above) will be acknowledged as the eligible number(s) for this event.
9. Employees or family members of employees of the sponsor, the client, American Hole 'n One, or any other agencies involved in this promotion in any manner are not eligible to participate as a contestant in this event.
10. The client or a representative of the client must supervise this event. It is the responsibility of this contest supervisor to ensure that all contestants are aware of all terms and conditions pertaining to the administration of this event. It must also be understood by all parties that a claim may subject a potential winning contestant and/or contest supervisor to a polygraph test given by a professional polygraphist in order to further verify the claim. A polygraph exam that reveals any fraudulent activity or non-compliance with any of the terms and conditions outlined herein may result in a claim denial.
11. Sponsor must provide AHNO with a video tape of this event. It is the responsibility of the sponsor representative to ensure that all video equipment used is in working order. The individual operating the video camera must record the event from start to finish, clearly showing the designated green (i.e 18th green) cleared, the Golf Balls being released from the helicopter (this can be done from the ground), the golf balls rolling around on the designated green. The video must zoom in to show each Golf Ball that is closest to the pin or in the cup, being removed from the cup, and held up so the camera can zoom in on the number printed on the Golf Balls. The entire sequence must be provided on continuous, unedited, uninterrupted video tape. In the event of a winner, this video must be delivered to AHNO within (5) business days of the contest event date. Failure to provide a video tape may result in a claim denial.
12. In the event of a cancellation, assuming no attempts to win the grand prize were initiated, client is entitled to a refund less a minimum $250 contract fee, or this contract may be amended to a rescheduled date without additional charges. American Hole ‘n One must be notified at least 24 hours in advance of any changes concerning the date(s) of this contest.
13. A winner is responsible for any and all tax consequences. American Hole ‘n One’ maximum aggregate liability for this contract is:
$25,000.00 Cash (maximum 1 winner(s)); per occurrence: $25,000.00 Cash, payable in the event that a contestant successfully selects the Golf Ball which corresponds with the predetermined winning number (in accordance with all terms and conditions outlined herein).
This contract covers a maximum of (1) total pick attempt(s) on each of the following date(s): 9/15/17

1. Caterina’s Club will conduct the contest on the above listed dates for a chance to win the following grand prize:
a. $25,000.00 Cash
2. Contest can be entered at Strawberry Farms Golf Club beginning 9/15/2017. Contestants will make a donation to
select a Golf Ball(s). There will be a maximum of 1,000 Golf Balls. Each Golf Ball will have a number printed on it
(from 001-1,000), each number will only be represented once. If the Golf Ball is dropped from the helicopter and
comes to rest closest to the pin or inside the cup of the designated hole on the golf course, the corresponding
contestant will have a chance to match the (1) predetermined winning number in the sealed prize envelope. Only the
one Golf Ball which has come to rest closest to the pin or inside the cup of the designated hole on the golf course
will be eligible. If the contestant’s Golf Ball number matches with the exact predetermined winning number, that
contestant will win the grand prize stated above, subject to verification. There will be a maximum of (1) winners.
If no contestant’s Golf Ball number successfully matches the predetermined winning Golf Ball number, no
grand prize will be awarded.
3. If there is a grand prize winner, the winner must come to Caterina’s Club at a mutually agreed date and time at least
2 weeks prior to delivery of the grand prize.
4. The contest is open to persons of all ages. Employees of Caterina’s Club, other related sponsors and their immediate
families may not participate and are not eligible to win. The term “immediate families” includes spouses,
grandparents, parents, siblings, children, and grandchildren.
5. Persons who have won another contest or promotion of any kind from Caterina’s Club within 30 calendar days
before the contest are ineligible to win.
6. Difficulties with getting to locations to enter the contest are not the responsibility of Caterina’s Club.
7. As winner of the grand prize, you will be required to sign a release form before your prize will be awarded. You will
be responsible for all applicable title fees, registration fees, and taxes including state, local, and federal and therefore
must present a valid Social Security Card before claiming your prize.
8. As a grand prize winner, you acknowledge that American Hole ‘n One and Caterina’s Club have the right to, without
further authorization, publicize your name, character likeness, photograph, voice, and the fact that you are a grand
prize winner for promotional purposes without financial remuneration.
9. There is no purchase necessary to enter or win the contest. Void where prohibited.
10. There will be no substitution for prizes. Participation in the contest and prizes awarded are non-transferable. Failure
to collect the prize constitutes forfeiture of the prize. If forfeited, the prize will remain the property of Caterina’s
Club. Another winner will not be selected.
11. Caterina’s Club reserves the right to make rule or contest changes without notice at any time and reserve the right to
change the aforementioned contest date without notice.
12. The decision of Caterina’s Club management in the interpretation of these rules is final. By participating, persons
agree to be bound by the decisions of the judges and the official contest rules.
13. If the winner is found in violation of these rules he/she may be required to forfeit his/her prize.
14. Caterina’s Club, their representatives, agencies and employees, are not responsible for any liabilities arising directly
or indirectly from the award or use of this prize.
15. Caterina’s Club, their representatives, agencies and employees, are not responsible for any changes or difficulties in
travel or prize delivery.
16. Odds of winning are 1 in 1,000. The chance to win the grand prize includes:
Contestant will select a numbered Golf Ball(s). There will be a maximum of 1,000 Golf Balls. Each Golf Ball will
have a number printed on it, each number will only be represented once (001-1,000). If the Golf Ball is dropped
from the helicopter and comes to rest closest to the pin or inside the cup of the designated hole on the golf course,
the corresponding contestant will have a chance to match the number on the Golf Ball to the predetermined winning
Golf Ball number. Only one (1) Golf Ball which has come to rest closest to the pin or inside the cup of the
designated hole on the golf course will be eligible. If the contestant’s Golf Ball number successfully matches with the
winning number, that contestant will win the grand prize, subject to verification. The prize does not cover any other
expenses, including but not limited to tax. Caterina’s Club is not responsible and will provide no warranties
regarding the use of the grand prize. Other restrictions may apply.
